Introducción: El consumo de sustancias psicoactivas representa un desafío para la salud sexual masculina debido a la heterogeneidad de la evidencia disponible sobre su impacto específico. Objetivo: Analizar la relación entre dichas sustancias y las disfunciones sexuales en hombres adultos. Método: Se llevó a cabo una revisión sistemática siguiendo la guía PRISMA 2020 en las bases PubMed, Scopus y Web of Science, abarcando publicaciones entre 2015 y 2025. Tras aplicar los criterios de elegibilidad, se incluyeron siete estudios cuantitativos que permitieron una síntesis narrativa de los hallazgos. Resultados: Los principales hallazgos indicaron que el alcohol se asoció significativamente con disfunción en el 64,1% de los dependientes, mientras que la metanfetamina mostró una prevalencia del 68,1% y correlación negativa con la prolactina. Asimismo, los antidepresivos serotoninérgicos (OR=3,2) y las benzodiacepinas (OR=2,14) aumentaron el riesgo, especialmente con insomnio coexistente, whereas el cannabis no presentó asociación significativa. Conclusión: Estos hallazgos sugieren que el deterioro sexual varía según el tipo de sustancia y la severidad del consumo. No obstante, la predominancia de diseños transversales limita la inferencia causal. Por consiguiente, se recomienda integrar la evaluación del consumo en la práctica clínica sexual y promover investigaciones longitudinales en Latinoamérica para comprender mejor los factores culturales y temporales implicados.

Introduction: Psychoactive substance use represents a challenge to male sexual health due to the heterogeneity of the available evidence regarding its specific impact. Objective: To analyze the relationship between psychoactive substance use and sexual dysfunction in adult men. Method: A systematic review was conducted following the PRISMA 2020 guidelines using the PubMed, Scopus, and Web of Science databases, covering publications from 2015 to 2025. After applying the eligibility criteria, seven quantitative studies were included, allowing for a narrative synthesis of the findings. Results: The main findings indicated that alcohol use was significantly associated with sexual dysfunction in 64.1% of individuals with alcohol dependence, whereas methamphetamine use showed a prevalence of 68.1% and a negative correlation with prolactin levels. Likewise, serotonergic antidepressants (OR = 3.2) and benzodiazepines (OR = 2.14) increased the risk, particularly in the presence of co-occurring insomnia, whereas cannabis use showed no significant association. Conclusion: These findings suggest that sexual impairment varies according to the type of substance and severity of use. However, the predominance of cross-sectional designs limits causal inference. Therefore, integrating substance use assessment into clinical sexual health practice and promoting longitudinal research in Latin America are recommended to better understand the cultural and temporal factors involved.
